When it comes to thyroid health, many people think only of iodine. But in my experience working with women over the years, I’ve seen time and again that a range of nutrients, environmental exposures, and even food choices can all play a significant role in how well the thyroid is functioning.

It’s not uncommon for patients to feel like something is off, even when their blood tests are technically within range. And sometimes, by identifying and correcting simple deficiencies, like selenium, iron, or vitamin D, we’ve been able to improve thyroid function naturally and even reduce reliance on high-dose levothyroxine. Here’s what you need to know.

Selenium
Selenium is a trace mineral that the thyroid relies on for two key functions: producing active thyroid hormone (T3) and protecting itself from inflammation. Low selenium is common and can be a trigger for autoimmune thyroid issues like Hashimoto’s.

Selenium is found in foods like Brazil nuts (1–2 per day is usually plenty), eggs, seafood, chicken, and sunflower seeds. In some people, supplementation is needed, but it must be done carefully, as too much can be harmful. This is something we review on a case-by-case basis.

Iodine
Iodine is essential for thyroid hormone production. Most people in the UK get enough through a varied diet, though dairy-free or vegan diets may reduce intake. Seaweed, white fish, shellfish, and eggs are good sources.

However, it’s not just about getting enough iodine, too much of certain other compounds (like fluoride, bromine, or raw cruciferous vegetables) can block iodine’s effect in the thyroid. We sometimes see this in people who have good iodine levels but still have symptoms.

Bromine
Bromine is in the same chemical family as iodine and can compete with it in the thyroid gland. It’s found in some soft drinks, baked goods, fire retardants, and pool chemicals. If your iodine intake is optimal, bromine likely won’t be an issue, but if iodine is low, excess bromine may affect hormone production.

Reducing processed foods and using non-toxic household and personal care products can help lower your exposure.

Filtered Water (Why This Matters in the UK)
In some countries, arsenic exposure from private well water is a real concern. In the UK, this is rare, but that doesn’t mean our tap water is perfect. Chlorine, microplastics, and trace contaminants can add to your overall toxic load.

Many of my patients choose to use a high-quality water filter at home, to reduce everyday exposures that can subtly affect thyroid and hormone health.

Iron
Your body needs iron to convert T4 (inactive thyroid hormone) into T3 (the active form). We often check ferritin (your iron storage marker), and if it’s below 80 µg/L, it may affect how well your thyroid is working.

Red meat, lentils, and leafy greens are good sources, and combining these with vitamin C (like lemon juice or red pepper) improves absorption.

Zinc
Zinc supports thyroid hormone production and immune balance. It also helps with wound healing, skin, and energy. If you’ve been under stress or have skin/hair issues, this could be relevant.

Pumpkin seeds, meat, shellfish, and legumes like chickpeas are great food sources.

Tyrosine
Tyrosine is an amino acid and a building block for thyroid hormones. Low protein diets or high stress may reduce its availability. Turkey, eggs, dairy, almonds, and tofu all provide tyrosine.

Vitamin D
Vitamin D helps regulate the immune system, which is particularly important in autoimmune thyroid conditions. In the UK, many people are deficient, especially in winter.

We test and supplement if needed, aiming for optimal, not just “normal”, levels.

Toxic Metals: Arsenic, Mercury, and Cadmium
Some environmental toxins can bind to nutrients like selenium and interfere with thyroid function. Arsenic (found in rice, seafood, wine), mercury (in large fish like tuna), and cadmium (from cigarettes or certain grains) all contribute.

These are things we test for when relevant, especially in patients with unexplained fatigue, hormone issues, or autoimmune thyroid disease.

Optimising thyroid health isn’t just about chasing a diagnosis or increasing medication. It’s about understanding the wider picture: what you eat, how your body absorbs and uses nutrients, your environment, and your individual symptoms.

In clinic, we use testing where needed, but also look at your diet, stress levels, and supplement use. For some women, addressing these root causes has made a huge difference in how they feel, and has even reduced the need for high-dose thyroid medication.

Bloods with GP – normal!
TSH (thyroid stimulating hormone) checked and normal!
Full blood count checked but not a ferritin level

We checked a full female hormone profile, Thyroid panel, including T4, T3 and autoantibodies, ferritin. In addition we checked selenium, iodine, zinc, toxins by doing a simple functional medicine test to look at urinary excretion of these amongst others! This showed a low normal T4 and T3 level, low ferritin (although in ‘normal range’), very low level of selenium and slightly high mercury levels. We took a holistic, root cause approach, and without treating with Levothyroxine, we were able to normalise this lady’s bloods and more importantly, improve her symptoms.

While this is still rare, it reinforces why supervision and testing are non-negotiable.

  • Always start with baseline bloods (liver, kidney, thyroid, inflammation, HbA1c, cholesterol, nutrients)
  • Review your digestive and gallbladder history
  • Monitor for symptoms like upper abdominal pain, nausea, or bloating

When Is the Best Time to Test Your Hormones?

One frequent question we get from patients at our clinic is about the optimal timing for hormone testing. This can vary depending on several factors, such as whether you have regular menstrual cycles, the length of your cycles, and whether you are approaching menopause.

Here at The Bespoke Clinic, we often use the DUTCH Plus test to provide a comprehensive hormone profile and information about adrenal health. Understanding when to test is crucial to getting accurate and useful results.

The typical menstrual cycle – see image

The menstrual cycle begins on the first day of the period, which also marks day one of the cycle. At this stage, both oestrogen and progesterone levels are quite low. As the cycle progresses, oestrogen levels start to climb, peaking just before ovulation. After the egg is released from the ovary, the corpus luteum forms from the leftover follicle and begins to produce progesterone. This increase in progesterone prepares the body for a potential pregnancy, supporting the uterine lining in case the egg is fertilized. The term progesterone itself hints at its role: PRO-GEST(ation), essential for maintaining a pregnancy. If no fertilization occurs, the corpus luteum breaks down, leading to a drop in both progesterone and oestrogen levels, and the cycle resets with the onset of another period.

Testing During a Regular Cycle

For women with regular monthly cycles, the ideal time to conduct a test is during the mid-luteal phase, roughly 5-7 days post-ovulation. In a typical 28-day cycle, this would correspond to days 19 to 22. This phase is significant because it’s when progesterone levels peak and oestrogen reaches the high point of its luteal plateau. This timing can offer invaluable insights into your menstrual health and hormonal balance.

Here’s a brief overview of what these results might indicate:

  • Low Progesterone with Normal or Low Estrogen: This might suggest that ovulation hasn’t occurred.
  • Normal Progesterone with Low Estrogen: Ovulation likely occurred, but the lower estrogen levels could affect the quality of the endometrial lining and egg, potentially impacting implantation.
  • High Estrogen with Normal Progesterone: Ovulation occurred, but there may be a dominance of estrogen, which can lead to symptoms like migraines, PMS, breast tenderness, and heavier menstrual bleeding.
  • Both Hormones Within Normal Range: Ovulation has occurred, indicating a typically healthy cycle.

Adjusting for Different Cycle Lengths

If your cycle is shorter or longer than the standard 28 days, we’ll adjust the testing day accordingly to align with your specific luteal phase. For instance, if you have a 25-day cycle, the collection would likely be between days 16 and 19; for a 35-day cycle, it would be around days 26 to 28.

Peri-Menopausal Women and Irregular Cycles

For those experiencing perimenopause or irregular cycles, using an ovulation predictor kit (OPK) can help determine the best time to collect your sample. Once the OPK indicates that ovulation is imminent, count forward 5-7 days to find your optimal testing window. Alternatively, the DUTCH Cycle Mapping Test might be recommended, where you would provide samples nearly every morning of your cycle. This method allows us to chart the fluctuations of your hormones throughout the month, offering a detailed view of your hormonal health.

A personal note from a counsellor

One of the best things about my job is you!

Being able to sit with you and be present during your journey is such a privilege. Taking the first step through the door to therapy is a big moment for many and I appreciate how hard that can be.  It can be a step into the unknown in terms of what to expect, understanding what will happen during sessions and also for how long it will take.

Counselling is not for everyone and that’s fine. So many people come to sessions initially thinking it will be a quick chat with a new found friend but it’s never that, it’s a lot more. Being able to accept help and support is such a positive step to better health and wellbeing and is an acknowledgement to knowing what you want for a happy healthy you.

There are many types of counsellors and therapists so finding the right one is a worthwhile search.  The best is a recommendation from someone you know or through a trusted source be that a GP, clinic or work.  Once you have found the right one hang onto them. Life has a way of throwing curve balls when you least expect it so knowing a good counsellor can be key to staying on track with good wellbeing and staying grounded.

As a qualified counsellor I have helped people through anxiety, depression, domestic abuse, low self-worth as well as grief, loss, suicide and trauma.  However, my main driver of late is women’s mental health and wellbeing with a focus on the many changes women go through. Me being one of them.

Menopause, menstrual cycle and hormonal changes can have a detrimental effect on women’s mental wellbeing.  You see and feel changes that you aren’t mentally prepared for and understanding those changes can be difficult.  We can all too often feel mentally younger than our actual age and sometimes we aren’t prepared for the inevitable physical changes that age brings. What you see in the mirror may not be what you see inside your head.

My hope is that when you walk through the door, we can gain a trusting therapeutic relationship that works for you.  I can help you understand your thoughts and feelings around these changes. And when our sessions end, you can cope better with them going forward. That’s it in a nutshell but as you guessed it, it can be challenging along the way.

Most of my clients who come along have no idea what they want. Some feel better just for getting things off their chest however, if I had to sum it up, I’d say it’s acceptance of who they are. Acceptance of changes outside of their control and learning to rub along with those changes, as well as gaining knowledge of how to live better with them.

Regardless of your reasons for coming along, I can promise you I will listen, I will be present, and I will try and help you find your answers and hopefully you’ll feel in a better place than you did beforehand.
