Low Testosterone & Men's Hormone Health

Testosterone levels do naturally decline with age, however symptoms are not always caused by testosterone alone. Sleep quality, stress, weight gain, nutrition, insulin resistance, thyroid function, medication use, alcohol intake and underlying health conditions can all influence how a man feels.

Many men notice changes in their energy, motivation, fitness, body composition, sleep or libido as they get older. These symptoms are often put down to ageing, a busy lifestyle, stress, or simply “getting older”, but there may be more to the story.

At The Bespoke Clinic, we take a broader, doctor-led approach to men’s health. Rather than focusing solely on testosterone replacement therapy (TRT), we aim to understand the factors contributing to your symptoms and help you optimise your overall health, wellbeing and long-term resilience.

Andropause

COMMON SYMPTOMS

Symptoms may include:

Persistent tiredness or fatigue

Burnout and reduced resilience to stress

Poor sleep quality

Reduced motivation or confidence

Brain fog or memory problems

Reduced libido

Erectile difficulties

Loss of muscle mass or strength

Increased body fat, particularly around the abdomen

Slower recovery from exercise

Anxiety or low mood

WHY DO THESE SYMPTOMS OCCUR?

Whilst testosterone can play an important role, many men are surprised to discover that symptoms can also be linked to:

We also sometimes see men whose testosterone levels appear reasonable on paper, but who are converting a significant proportion of that testosterone into oestrogen. This process, known as aromatisation, can be influenced by body fat, alcohol intake, inflammation, metabolic health and liver function.

As a result, some men develop symptoms typically associated with low testosterone despite having testosterone levels within the normal range. Understanding this balance can be just as important as measuring testosterone itself.

A DOCTOR-LED ASSESSMENT

During your consultation, we will discuss your symptoms, medical history, lifestyle, exercise habits, sleep, nutrition and overall health goals.
Where appropriate, we may recommend blood testing to assess:

Our aim is to understand what is driving your symptoms and identify the most appropriate approach for you.

DO ALL MEN WITH LOW TESTOSTERONE NEED TRT?

No.

Many men see significant improvements through optimising sleep, stress management, nutrition, exercise, body composition and overall metabolic health.

Where testosterone deficiency is confirmed and treatment is appropriate, testosterone replacement therapy (TRT) may be discussed as part of a wider management plan.

The goal is not simply to increase a hormone level on a blood test, but to help you feel healthier, stronger and more resilient in everyday life.

OUR APPROACH

At The Bespoke Clinic, we combine conventional medical assessment with a functional medicine perspective.

This means we not only consider hormone levels, but also look at the wider factors that influence men’s health, including sleep, stress, nutrition, gut health, inflammation, metabolic health and lifestyle.

For some men, this may involve lifestyle changes and nutritional support. For others, it may include further investigation, hormone optimisation or testosterone replacement therapy where clinically appropriate.

CASE STUDY

A gentleman in his early fifties attended the clinic concerned about low energy, increasing abdominal weight gain, poor motivation and reduced performance in the gym. He assumed low testosterone was the cause and was keen to explore TRT.

Following a detailed assessment, several contributing factors were identified including poor sleep quality, high stress levels and metabolic health concerns. by handling those issues and reviewing his detox pathways, we were actually able to quickly improve his symptoms without the use of TRT.

Frequently, the answer is more nuanced than simply prescribing a hormone.

Questions answered

Frequently asked questions.

What is low testosterone?

Low testosterone, sometimes referred to as testosterone deficiency or andropause, occurs when testosterone levels fall below the level required for optimal health and wellbeing. Symptoms can include fatigue, reduced libido, erectile difficulties, loss of muscle mass, low mood and brain fog.

Andropause is a term sometimes used to describe age-related hormonal changes in men. Unlike menopause, testosterone levels tend to decline gradually over many years rather than stopping suddenly.

Symptoms alone cannot confirm low testosterone. A proper assessment and appropriate blood testing are required to determine whether hormone levels are contributing to your symptoms.

Yes, where clinically appropriate. Treatment decisions are based on symptoms, medical history, examination and blood test results. Not everyone with symptoms will require TRT.

In many cases, yes. Improving sleep, reducing excess body fat, increasing resistance training, managing stress and optimising nutrition can all help support healthy hormone production

Low testosterone is often associated with wider metabolic health issues such as increased body fat, insulin resistance and reduced exercise capacity. This is one reason why a comprehensive assessment is important.
