What is Testosterone Replacement Therapy (TRT)?

Testosterone Replacement Therapy (TRT) is prescribed to men with low testosterone levels to help improve energy, mood, muscle mass, fat metabolism, libido, and overall well-being. Low testosterone, or hypogonadism, can be due to ageing, chronic stress, metabolic conditions, or other underlying health issues.

Why a Compounded Testosterone Cream?

At The Bespoke Clinic, we use a compounded testosterone cream applied to the scrotal area, which offers a steady absorption rate and is generally well tolerated. This method ensures more stable blood levels of testosterone compared to injectable forms.

How to Apply Testosterone Cream

  • Apply once daily at the same time each day.
  • Wash and dry the scrotal skin before application.
  • Use the prescribed dose, spreading it thinly and allowing it to fully absorb before dressing.
  • Wash hands immediately after application.
  • Avoid skin contact with others for at least 4 hours after application.

Monitoring & Follow-Up

Regular blood tests are required to monitor your response to TRT and assess any potential risks. These include:

  • Full Blood Count (FBC) – to check for changes in red blood cell levels (polycythaemia risk).
  • E2 (Oestradiol) Levels – to ensure appropriate testosterone conversion and prevent excessive oestrogen-related effects.
  • Prolactin – to rule out underlying hormonal imbalances.
  • Liver Function Tests (LFTs) – to monitor liver health.
  • PSA (Prostate-Specific Antigen) – to screen for prostate health.
  • DHEA-S (Dehydroepiandrosterone Sulfate) – an important precursor hormone.

Your blood levels will be checked at baseline and periodically thereafter, with adjustments made based on your response and test results.

Expected Benefits of TRT

  • Increased energy and reduced fatigue
  • Improved mood and motivation
  • Enhanced libido and sexual function
  • Increased muscle mass and strength
  • Reduction in body fat
  • Improved cognitive function

Potential Side Effects & Risks

While TRT is generally well tolerated, potential side effects include:

  • Skin reactions (redness or irritation at the application site)
  • Increased red blood cell count (polycythaemia), which may require monitoring and management
  • Conversion of testosterone to oestrogen, leading to fluid retention or breast tissue changes (managed with dose adjustments if needed)
  • Acne or oily skin
  • Changes in mood or sleep
  • Prostate health concerns (hence PSA monitoring)

Dietary & Lifestyle Support for Optimal Testosterone Levels

While TRT can be highly beneficial, optimising your diet and lifestyle helps support testosterone production and metabolism. Key nutrients include:

  • Zinc (oysters, beef, pumpkin seeds, lentils)
  • Magnesium (spinach, almonds, dark chocolate, avocado)
  • Boron (raisins, walnuts, prunes, chickpeas)
  • Vitamin D (fatty fish, egg yolks, fortified dairy, sunlight exposure)

Lifestyle recommendations:

  • Prioritise strength training and regular exercise
  • Maintain good sleep hygiene (7-9 hours per night)
  • Manage stress through mindfulness, relaxation, and breathwork
  • Avoid excessive alcohol and processed foods
